In my last post, I discussed the newest GLP-1 medication, Foundayo, a once-daily oral version that will make weight loss feel more accessible than ever.
GLP-1s help people eating less without trying. And now with the oral options, cravings can be easily conquered. The constant mental chatter around food starts finally fades. For many, it’s the first time in years that weight loss doesn’t feel like a daily battle of willpower.
On paper, GLP-1 medications seem like an obvious winner. After years, the scale finally moves in the right direction, clothes fit more loosely, and confidence gets a boost. What’s not to love?
But this is where the story usually gets oversimplified.
GLP-1s don’t “fix” appetite. They suppress it. That distinction matters more than most people realize, because when the medication stops, appetite typically returns right along with it. And when appetite returns, weight regain often follows. For many individuals, this means these medications aren’t a short-term solution; they’re a long-term, sometimes lifelong tool.
That’s the first reality check.
The second one is less talked about, but arguably more important: even when the weight comes off successfully, not all of it is coming from fat. A meaningful portion can come from lean muscle, which changes how your body looks, feels, and functions long after the number on the scale drops.
The Muscle Loss Nobody Warns You About
When people think about weight loss, they assume they’re losing body fat. And yes, that’s part of it. But physiologically, weight loss is rarely that selective.
Clinical data on GLP-1 medications like Ozempic and Wegovy show that a substantial percentage of total weight loss—often estimated around 25–40%—can come from lean body mass, especially in the absence of resistance training or adequate protein intake. This isn’t unique to GLP-1s; it happens with most forms of rapid weight loss, but the appetite-suppressing nature of these drugs can amplify the effect.
In the STEP trial (from 2021; that’s old as dirt relatively speaking for GLP-1 studies) review published in the New England Journal of Medicine, participants taking semaglutide lost both fat mass and lean body mass. While fat loss made up the majority of the weight reduction, lean mass still decreased in absolute terms, confirming that muscle loss is part of the equation, especially without targeted strategies to preserve it.
Muscle is not just cosmetic tissue. It’s metabolically active, meaning it plays a direct role in how many calories you burn at rest, how well you regulate blood sugar, and how resilient your body is to stress, injury, and aging. When muscle is lost, those systems don’t just stay the same, they often decline.
Why GLP-1 Weight Loss Often Includes Muscle Loss
GLP-1 medications work by slowing gastric emptying and increasing satiety signals, which leads to a significant reduction in calorie intake. For many people, that reduction is dramatic. Meals become smaller, hunger cues are blunted, and overall intake can drop well below what the body is used to.
From a weight loss perspective, that’s the goal. However, from a physiology standpoint, it creates a trade-off.
Your body doesn’t inherently prioritize fat loss over muscle preservation. It responds to energy availability and demand. If energy intake drops and there’s no consistent stimulus telling the body to maintain muscle, it will begin to break down lean tissue as part of the overall weight loss process. Low protein intake only accelerates this, because the body lacks the building blocks it needs to maintain muscle mass.
In other words, muscle loss isn’t a side effect, but rather a predictable outcome when the right countermeasures aren’t in place.
When Weight Loss Doesn’t Feel Like a Win
This is where expectations and reality start to diverge.
On paper, losing 20 or 30 pounds looks like success. But in practice, some people notice they don’t feel stronger, more energized, or more confident in their bodies. Instead, they feel a bit depleted, and their body composition doesn’t reflect the lean, toned look they had in mind.
If weight loss includes a significant amount of muscle loss, the result is often a body that is smaller but not necessarily healthier or more functional. Yes, the number on the scale goes down, but so does metabolic capacity, strength, and overall resilience. Over time, that can make maintaining weight loss more difficult, not less.
Resistance Training: The Easiest Way To Prevent GLP-1 Muscle Wasting
This is where resistance training becomes essential, not optional.
When you lift weights or perform any form of progressive resistance exercise (example: pushups on your knees), you’re sending a clear message to your body: this tissue is needed. That signal changes how your body allocates resources during a calorie deficit, helping to preserve lean mass even as fat is being lost.
The benefits extend beyond muscle preservation. Strength training improves insulin sensitivity, supports bone density, and helps maintain a higher resting metabolic rate. These are all important during weight loss, especially for women navigating hormonal changes.
The good news is that you don’t need an extreme Crossfit program to see results. Instead, follow this simple, consistent approach works:
- 2–4 strength sessions per week
- Focus on compound movements like squats, hinges, pushes, and pulls
- Gradually increase resistance or repetitions over time
That’s enough to create the stimulus your body needs to hold onto muscle.
Protein Intake: The Overlooked Variable
Even with resistance training in place, protein intake plays a critical role.
GLP-1 users often eat less across the board, and because protein is highly satiating, it’s frequently under-consumed. This creates a situation where the body is in a calorie deficit without adequate amino acids to support muscle maintenance.
A practical target for most individuals is approximately 0.7 to 1.0 grams of protein per pound of goal body weight. That range helps support muscle retention during weight loss without requiring extreme dietary changes.
It doesn’t have to be perfect, but it does need to be intentional. Otherwise, the combination of low calories and low protein becomes a setup for continued muscle loss.
The Reality for Women in Midlife
For women in their 40s and beyond, this conversation carries even more weight.
Age-related muscle loss, declining estrogen levels, and increasing insulin resistance are already in play. These shifts make it easier to lose muscle and harder to rebuild it. When rapid weight loss is layered on top without resistance training and adequate protein, the risk of ending up lighter but weaker increases significantly.
