The Controversy Around Adrenal Fatigue
If you’ve mentioned “adrenal fatigue” to your conventional doctor, you may have been told it isn’t a real diagnosis. They’re not entirely wrong — and they’re not entirely right either.
“Adrenal fatigue” as a clinical term isn’t recognized in mainstream endocrinology. But the underlying physiology it describes — a dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis under chronic stress — is very real and very well documented.
The more precise term is HPA axis dysfunction or cortisol dysregulation. And it’s not about your adrenal glands being “fatigued” — it’s about the entire stress response system becoming dysregulated after prolonged activation.
How the HPA Axis Is Supposed to Work
When you perceive a stressor, your hypothalamus signals the pituitary gland, which signals the adrenal glands to release cortisol. Cortisol mobilizes energy, suppresses inflammation, and sharpens focus — all adaptive in the short term.
The problem is that this system evolved for acute, short-term stressors — not the chronic, relentless stress of modern life. Work deadlines, financial strain, relationship tension, poor sleep, blood sugar swings, and even chronic infection all activate this same axis, day after day.
Over time, the feedback loops that keep cortisol in balance can become blunted. Cortisol may be too high at the wrong times (usually at night, disrupting sleep) and too low at the right times (morning, causing that “hit by a truck” feeling upon waking).
Symptoms of HPA Axis Dysfunction

The symptom picture often includes several of the following:
- Profound fatigue that sleep doesn’t resolve — particularly morning exhaustion
- Energy crashes in the early afternoon (2–4 pm is classic)
- Difficulty handling stress — feeling overwhelmed by things that used to feel manageable
- Brain fog and poor concentration
- Craving salty or sweet foods — especially in the afternoon
- Low blood pressure or dizziness when standing
- Sleep disruption — trouble falling asleep, waking at 2–4 am, or feeling unrefreshed despite adequate hours
- Frequent illness — immune function is closely tied to cortisol regulation
- Mood instability — anxiety, irritability, low motivation
- Weight gain around the abdomen — chronic cortisol elevation promotes visceral fat accumulation
How We Test for Cortisol Dysregulation
Standard blood cortisol testing typically misses HPA axis dysfunction because it only captures a single point in time. Functional medicine uses more nuanced approaches:
- 4-point salivary cortisol: Samples taken upon waking, morning, afternoon, and evening map your cortisol rhythm throughout the day. This reveals patterns — like a flat curve, an inverted curve, or specific dips — that a single blood draw cannot.
- DUTCH Complete test: Dried urine testing for comprehensive hormones measures cortisol, cortisone, and their metabolites, along with sex hormones and melatonin. It’s the most comprehensive picture available.
- DHEA-S: This adrenal androgen often declines in chronic stress and is a useful complementary marker.
The Functional Medicine Approach to Treatment
There’s no single pill that fixes HPA axis dysfunction. Recovery requires addressing the load on the system — which means identifying and removing stressors, supporting the body’s physiological needs, and rebuilding resilience over time.
Nutritional Support
- Blood sugar stabilization is foundational — cortisol spikes in response to blood sugar drops. Eating protein with every meal, avoiding refined carbohydrates, and not skipping meals are first steps.
- Adequate sodium intake — counterintuitively, many people with HPA dysfunction feel better with more high-quality salt
- Anti-inflammatory foods to reduce the inflammatory load the adrenal system must manage
Targeted Supplementation
- Adaptogens: Ashwagandha, rhodiola, holy basil, and eleuthero have the most evidence for HPA axis support
- Phosphatidylserine: Shown in clinical trials to reduce cortisol response to stress
- Magnesium: Critical for over 300 enzymatic reactions; depleted rapidly under stress
- Vitamin C: The adrenal glands have the highest concentration of vitamin C in the body
