When you start researching non-surgical fat reduction, you quickly realize there are a lot of options — and a lot of marketing claims. Three treatments come up most often: CoolSculpting, liposuction, and the Emerald Laser.
They’re all different. They work through completely different mechanisms, carry different risk profiles, suit different candidates, and cost very different amounts. The “best” one depends entirely on your body, your goals, your BMI, and what tradeoffs you’re willing to accept.
I’m going to give you a straight comparison — the good and the limitations of each — so you can make an informed decision. And yes, I’ll be upfront: we offer the Emerald Laser at Fundamental Wellness. But I’m going to give you an honest picture of all three.
A Quick Overview of Each Treatment
The Emerald Laser
The Emerald Laser is a low-level laser therapy (LLLT) device that uses 10 green diode lasers to penetrate fat cells and create temporary pores in the cell membrane. Stored fat drains out and is processed by your lymphatic system. Fat cells are preserved — just emptied.
It’s FDA-cleared (K192544) for circumference reduction in patients with a BMI up to 40.
CoolSculpting
CoolSculpting (cryolipolysis) uses controlled cooling to freeze fat cells to the point of destruction. Over the weeks following treatment, your body absorbs and processes the dead cells. Results can be significant, but the treatment kills fat cells rather than emptying them.
It’s FDA-cleared for several body areas, but generally suited for patients with lower BMIs.
Liposuction
Liposuction is a surgical procedure in which a cannula (thin tube) is inserted through small incisions and fat is physically suctioned out. It is the most aggressive option, produces the most dramatic results, but requires anesthesia, carries surgical risks, and involves recovery time.

The Safety Question: What Is Paradoxical Adipose Hyperplasia (PAH)?
This is the issue that most people haven’t heard about with CoolSculpting, and it’s worth understanding.
Paradoxical adipose hyperplasia (PAH) is a rare but documented complication where the treated area develops a hard, enlarged growth of fat instead of shrinking — the opposite of the intended result. It’s estimated to occur in about 1 in 3,000 to 1 in 20,000 treatments, though some studies suggest higher rates.
PAH is thought to occur when the freeze triggers abnormal fat cell proliferation rather than death. It can require surgery to correct.
The Emerald Laser has no documented PAH risk because it doesn’t freeze or destroy cells. The mechanism is fundamentally different — it empties fat cells rather than killing them.
This isn’t a reason to avoid CoolSculpting automatically. PAH is rare. But if you’re someone who doesn’t want that risk on the table at all, the Emerald Laser is the more conservative choice.
The BMI Factor: Why It Actually Matters
One of the most clinically important differences between these treatments is BMI eligibility.
Most non-invasive fat reduction devices — including CoolSculpting — are designed for and tested in patients with a lower body mass index. The Emerald Laser is specifically FDA-cleared for patients with a BMI up to 40.
This matters enormously in a real-world clinical practice. Many of the patients who most want non-invasive fat reduction — people actively working on their weight, or those who struggle with localized fat deposits despite exercising — have a BMI above the threshold that most devices were tested for.
The Emerald Laser was designed to serve this population. It was specifically studied in and cleared for higher-BMI patients. If your BMI is in the 30s or low 40s, the Emerald Laser may be the only non-invasive option that was actually tested on people like you.
When Is Liposuction Actually the Better Choice?
I’m a functional medicine NP, and I’m honest with my patients: sometimes surgery is the right answer.
Liposuction is the better choice when:
- You want dramatic, permanent results from a single procedure
- You’re treating large-volume fat deposits that don’t respond well to non-invasive approaches
- You need skin tightening alongside fat removal (some lipo techniques address both)
- You’re already planning surgical procedures and want to combine treatments
- You don’t mind the recovery time and anesthesia risks
Liposuction permanently removes fat cells from treated areas. If you gain weight after, it tends to distribute to other areas. The results can be more dramatic and more immediate than any non-invasive option.
The tradeoffs are real: cost ($5,000–$12,000+), surgical risks (infection, complications, uneven contours), and recovery time (weeks of activity restriction). But for some patients, the math works.
When the Emerald Laser Makes the Most Sense
The Emerald Laser is the right choice when:
- You want a non-invasive option with zero downtime
- Your BMI is in the 30–40 range
- You’re targeting stubborn localized fat that isn’t responding to diet and exercise
- You’re already working on your weight and want to accelerate contouring
- You want to pair fat reduction with coaching and health optimization
- You don’t want the financial, physical, or emotional weight of surgery
