If you live in Northeast Nebraska, chances are you are not exactly living a Wall Street, subway-at-midnight, coffee-in-one-hand, phone-in-the-other kind of life.

Norfolk is not New York City.

But that does not mean the clients I meet with at the Diabetes & Wellness Clinic are calm as a clam, perfectly balanced and blissfully drifting into eight or nine hours of uninterrupted sleep every night.

Between work, kids, grandkids, aging parents, school events, church commitments, night sweats, 3 a.m. wakeups, blood sugar swings, and the mental load of keeping everyone else’s life running, most women I talk to are not getting the recommended 8 hours.

So are we all doomed to age faster than we should?

Did prairie women in the 1850s get eight perfect hours after hauling water, tending fires, feeding families, birthing babies, preserving food, and surviving Nebraska winters? Probably not. We do not have reliable sleep-tracking data from women homesteading on the Great Plains, but common sense tells us their sleep was shaped by hard physical labor, daylight, weather, caregiving, and survival.

In 1942, 84% of U.S. adults told Gallup they were getting at least seven hours of sleep a night. By 2013, that dropped to 59%. And according to 2024 CDC/NCHS data, about 3 in 10 U.S. adults now average fewer than seven hours in a 24-hour period. So no, it is not just you, and it is not just Northeast Nebraska. A lot of us are sleep deprived. [See: Gallup “Eyes Wide Open” survey]

So maybe the better question is not, “Are you getting eight hours?” Instead, we should ask, “Am I getting enough quality sleep for my body to repair, regulate hormones, stabilize blood sugar, calm inflammation, and support healthy aging?”

8 Hours Or More of Sleep: Overrated?

That is where the newest research gets interesting.

A large study published in the journal Nature analyzed sleep data from roughly 500,000 people and found that the lowest biological aging signals were not necessarily seen in people sleeping nine or ten hours. The “sweet spot” was closer to 6.4 to 7.8 hours per night.

In other words, people in that sleep range tended to show fewer signs of accelerated aging.

So no, you may not need a perfect eight, nine, or ten hours every night.

But you do need enough consistent, restorative sleep for your body to repair, regulate hormones, stabilize blood sugar, calm inflammation, and support healthy aging.

How Does Sleep Affect Biological Aging?

Your chronological age is the number of birthdays you have had.

Your biological age is different. It’s how your cells, tissues, organs, metabolism, immune system, and nervous system are functioning compared to what might be expected for your actual age.

Two women may both be 52 years old, but one may have more inflammation, poorer blood sugar regulation (insulin resistance), higher stress hormones, lower muscle mass, more oxidative stress, or less cellular repair capacity.

That woman may be experiencing a faster rate of biological aging.

Researchers study biological aging through markers such as inflammation, immune function, metabolic health, telomere length (the end caps on DNA), mitochondrial function, and epigenetic changes.

One of the most interesting areas of aging research involves something called epigenetics.

Sleep and Epigenetics: Why Your Genes Need Rest

Epigenetics refers to the way your body turns genes (proteins) on or off without changing your actual DNA sequence.

Think of your DNA as the instruction manual.

Epigenetics is how your body highlights, bookmarks, silences, or activates certain instructions based on signals from your environment.

Those signals include nutrition, stress, toxins, exercise, inflammation, hormones, and sleep.

One of the most studied epigenetic processes is DNA methylation. This is a chemical tagging system that helps regulate gene expression. Patterns of DNA methylation change with age, and scientists now use some of these patterns to estimate “epigenetic age.”

When sleep is chronically disrupted, the body may receive a repeated signal of stress.

That can influence inflammation, cortisol rhythms, insulin sensitivity, immune signaling, oxidative stress, and cellular repair. Over time, those changes may contribute to a faster biological aging pattern.

This does not mean one bad night of sleep ages you overnight.

It means that chronic sleep disruption may become one more stressor pushing the body away from repair and toward survival mode.

Why Sleep Deprivation Feels Worse During Perimenopause and Menopause

Some of my female clients report not being able to recover from a poor night of sleep like they used to.

Then perimenopause begins, and suddenly one bad night feels like it takes three days to recover.

There is a reason for that.

Estrogen and progesterone influence sleep quality, body temperature regulation, mood, stress response, and nervous system balance. As these hormones fluctuate and decline, many women experience more nighttime waking, lighter sleep, hot flashes, night sweats, anxiety, early morning waking, and changes in circadian rhythm.

Progesterone has a calming effect on the nervous system. Estrogen helps influence serotonin, temperature regulation, and sleep architecture. When these hormones become unpredictable, sleep can become unpredictable too.

This is why many women in perimenopause say things like:

“I wake up at 3 a.m. and cannot fall back asleep.”

“I feel tired but wired.”

“I feel anxious at night.”

“I wake up hot, sweaty, or with my heart racing.”

“I am doing everything right, but my sleep is suddenly terrible.”

These are red flags that your nervous system, endocrine system, and/or circadian rhythms are struggling to adapt to hormonal change.

The Cortisol Connection

Cortisol is your main stress hormone. Despite its bad reputation, it helps wake you up in the morning, regulate energy, respond to stress, maintain blood pressure, and support blood sugar balance.

Ideally, cortisol rises in the morning and gradually declines throughout the day. By evening, your body should be shifting toward melatonin production, lower body temperature, and parasympathetic nervous system activity.

But chronic stress, blood sugar swings, alcohol, overtraining, under-eating, late-night screens, inflammation, and hormone changes can disrupt this rhythm.

This can lead to middle of the night wake-ups or craving sugar or caffeine the next day.

Then, the viscous cycle repeats day in and day out.

When this becomes chronic, sleep deprivation can worsen blood sugar regulation, increase inflammation, intensify cravings, reduce resilience, and make menopause symptoms feel harder to manage.

Sleep, Blood Sugar, and Weight Changes

Poor sleep can make it harder to regulate blood sugar.

During midlife hormonal changes, estrogen depletion affects insulin sensitivity, body composition, and where fat is stored.

When sleep is disrupted, the body may become more insulin resistant. Hunger hormones can shift. Cravings may increase. Late-night snacking may become more common. Energy may drop, making it harder to exercise or prepare nourishing meals.

This is why “just eat less and move more” often fails women in perimenopause and menopause.

If sleep is poor, the body may interpret that as stress.

And when the body feels stressed, it is less interested in fat loss and more interested in protection.

Better sleep does not magically fix every metabolic issue, but it often makes the rest of the plan work better.

How Sleep Supports Cellular Repair

During deep sleep, the body performs essential repair work.

This includes tissue recovery, immune regulation, hormone signaling, brain detoxification, memory consolidation, and mitochondrial maintenance.

Mitochondria are the energy-producing structures inside your cells. They are highly sensitive to stress, inflammation, nutrient deficiencies, and poor sleep.

When sleep is consistently poor, mitochondria may produce energy less efficiently. That can show up as fatigue, brain fog, poor exercise recovery, mood changes, and reduced resilience.

This is one reason sleep is so central to healthy aging.

You cannot pop supplements like candy or think that exercise and copious amounts of coffee can fix the problem.

Why “More Sleep” Is Not Always the Whole Answer

Many women are told to “just get more sleep.”

But that advice is not always helpful because life doesn’t afford that luxury. And when your hormones become imbalanced, that’s even more cards in the deck stacked against you.

All, or much of the above?

A functional medicine approach looks for the reason sleep is breaking down rather than treating it as an isolated symptom.

Getting good quality sleep doesn’t just make you feel well rested. It’s intricately connected to your hormones, gut, liver, thyroid, blood sugar, nervous system, immune system, and stress response.

Practical Ways to Support Sleep and Healthy Aging

Improving sleep does not always require a complicated protocol. Often, the biggest improvements come from restoring rhythm.

Start with a consistent wake time. Morning light helps anchor your circadian rhythm and tells your brain when the day begins.

Eat enough protein earlier in the day. Under-eating during the day can contribute to nighttime waking and blood sugar instability.

Be mindful with alcohol. Even if it helps you fall asleep, alcohol often fragments sleep later in the night.

Create a cooler sleep environment. This is especially important for women dealing with night sweats or hot flashes.

Reduce bright light and screens in the evening. Your brain needs darkness to support melatonin production.

Avoid intense late-night exercise if it leaves you wired.

Consider whether caffeine is lasting longer than you think. Many women become more sensitive to caffeine in midlife.

And if you are waking up gasping, snoring heavily, waking with headaches, or feeling exhausted despite enough hours in bed, it may be worth evaluating for sleep apnea.
