Why Hormones Matter More Than You Think
Somewhere in your late 30s or 40s, something shifts. Sleep gets harder. Weight accumulates around your midsection despite no change in diet. Your mood feels less stable. Brain fog creeps in. You’re told “this is just aging” — and you’re expected to accept it.
But that narrative is changing. What’s often dismissed as normal aging is frequently hormonal decline — a measurable, treatable physiological change. And hormone replacement therapy (HRT) has become one of the most evidence-backed tools available for addressing it.
What Is Hormone Replacement Therapy?
HRT involves supplementing hormones that the body produces in declining amounts — primarily estrogen and progesterone, and sometimes testosterone. It’s most commonly associated with menopause management, but it’s increasingly being used earlier, during perimenopause, when hormone fluctuations begin but periods haven’t stopped.
There are two broad categories:
- Synthetic HRT: Hormones that are chemically similar but not identical to those your body produces. Examples include conjugated equine estrogens (Premarin) and medroxyprogesterone acetate (Provera).
- Bioidentical HRT: Hormones that are molecularly identical to those produced by your body, derived from plant sources. These can be commercially manufactured (FDA-approved) or custom-compounded at a pharmacy.
The Bioidentical vs. Synthetic Debate

This is the question we get most often. The short answer: bioidentical hormones are generally preferred in functional medicine because their molecular structure matches what your body makes — which may affect how they’re metabolized and tolerated.
The 2002 Women’s Health Initiative (WHI) study that scared a generation of women away from HRT used synthetic, oral hormones — not bioidentical options. More recent research paints a far more nuanced picture, particularly when hormones are started within 10 years of menopause onset (the “timing hypothesis”).
At our practice, we favor bioidentical hormones delivered transdermally (through the skin) when possible, as this bypasses first-pass liver metabolism and maintains a more stable hormone level throughout the day.
Benefits of HRT: What the Evidence Shows
When prescribed appropriately and monitored carefully, HRT offers well-documented benefits:
- Hot flash and night sweat relief — estrogen is the most effective treatment available for vasomotor symptoms
- Improved sleep quality — both estrogen and progesterone influence sleep architecture
- Mood stabilization — estrogen has direct effects on serotonin and dopamine pathways
- Reduced brain fog and improved cognitive function
- Bone density preservation — estrogen is critical for preventing osteoporosis
- Cardiovascular protection — when started early in menopause, estrogen may reduce cardiovascular disease risk
- Vaginal and urinary health — local estrogen therapy dramatically improves genitourinary symptoms of menopause
- Libido improvement — especially with the addition of testosterone
Understanding the Risks
No medical intervention is without risk, and honest conversations about HRT require acknowledging them:
- Breast cancer: The relationship is complex. Estrogen-only HRT in women without a uterus does not appear to increase breast cancer risk and may actually lower it. Combined estrogen-progestogen therapy shows a small increased risk after 5+ years — though this is largely associated with synthetic progestins, not bioidentical progesterone.
- Blood clots: Oral estrogen carries higher clot risk than transdermal. Women with clotting history need careful evaluation.
- Stroke: Oral estrogen slightly increases stroke risk; transdermal does not appear to.
Absolute contraindications include active breast or uterine cancer, unexplained vaginal bleeding, and active blood clots. Women with a history of these conditions need individualized assessment.
Who Is a Candidate for HRT?
HRT may be appropriate for you if you:
- Are experiencing moderate to severe menopausal symptoms affecting quality of life
- Are under 60 or within 10 years of menopause onset (the timing window where benefits are clearest)
- Have been diagnosed with premature ovarian insufficiency (early menopause)
- Have significant bone loss concerns
- Are struggling with mood, sleep, or cognitive symptoms related to hormonal changes
