You know the one.

You’re with a group of girlfriends, and anytime somebody complains about anything, she says, “There’s a peptide for that.”

PMS problems? There’s a peptide for that.
Bloating? Yup, peptide.
Wrinkles? Check.
Slow workout recovery? Peptide.
Low libido? Also peptide.
Hangnail? Honestly, she can probably make a case for it.

And depending on what you are talking about, she may not be entirely wrong. Peptides are being studied and used for everything from appetite regulation and body composition to tissue repair, skin health, inflammation, hormone signaling, and sexual function.

GLP-1 medications like semaglutide and tirzepatide are peptide-based drugs. Growth hormone-related peptides such as sermorelin, CJC-1295, and ipamorelin are often used in body composition and recovery protocols. GHK-Cu offers skin and collagen support. PT-141, also called bremelanotide, is used for sexual desire concerns in certain patients.

So yes, peptides are real. They are not just TikTok wellness glitter.

But that is exactly why they should not be treated like casual supplements, mystery injections, or something you buy online because a friend, influencer, or biohacking forum swears by it.

Peptides are biologically active compounds. They send signals in the body. And when you send the wrong signal, use the wrong dose, inject the wrong product, or skip medical screening altogether, the risks become very real.

What Are Peptides?

Peptides are short chains of amino acids, which are the building blocks of protein. In the body, many peptides act like messengers. They bind to receptors and tell cells, glands, or tissues to take action.

Insulin is a peptide hormone. GLP-1 is a peptide hormone. Other peptides used in wellness and functional medicine may be designed to influence growth hormone release, tissue repair, inflammation, immune function, body composition, or skin health.

That “messenger” role is exactly why you need medical oversight. If a peptide can send a signal, it can also send the wrong signal, send too strong of a signal, or affect a system that should not be stimulated in your particular body.

The Biggest Risk: You May Not Know What You Are Actually Injecting

One of the most common problems with unsupervised peptide use is sourcing.

Many online peptides are sold as “research peptides” with labels like “not for human use.” Yet they are often marketed in ways that clearly suggest people are supposed to inject them. (Like medical marijuana and CBD/hemp of a few years ago, peptide therapy is in early Wild West days.)

This creates a major safety problem. Products sold outside legitimate medical and pharmacy channels may be:

  • Mislabeled
  • Underdosed
  • Overdosed
  • Contaminated
  • Improperly stored
  • Mixed with unstudied ingredients
  • Different from what the label claims

Injectable products bypass many of the body’s normal defenses. If a capsule is low quality, that is bad enough. And if an injectable product is contaminated, incorrectly concentrated, or poorly prepared, the risk is more serious.

Even compounded medications are not the same as FDA-approved medications. The FDA explains that compounded drugs do not go through FDA premarket review for safety, quality, or effectiveness and generally should be used only when a patient’s medical needs cannot be met by an available FDA-approved drug.

“I Followed the Instructions” Is Not Always Enough

One of the clearest examples of peptide-related danger comes from compounded GLP-1 medications.

Semaglutide is a GLP-1 receptor agonist, which means it is part of a peptide-based medication class. FDA-approved versions come in standardized pens or tablets with specific dosing instructions. But compounded injectable versions may come in vials with varying concentrations.

That creates room for error.

In 2024, the FDA reported adverse events, including some requiring hospitalization, related to dosing errors with compounded injectable semaglutide. Some patients accidentally gave themselves five to 20 times the intended dose. The FDA noted that confusion between milliliters, milligrams, and “units” on syringes contributed to some of these mistakes.

Reported adverse events included nausea, vomiting, abdominal pain, fainting, headache, migraine, dehydration, acute pancreatitis, and gallstones. In several cases, the error was not reckless behavior but rather confusion over vial concentration, syringe markings, or unclear dosing instructions.

In other words, even when people think they are being careful, injectable dosing can go wrong quickly without proper instruction.

Dosing Is Not One-Size-Fits-All

Peptide dosing depends on the specific compound, concentration, route of administration, patient history, current medications, lab values, goals, and risk factors.

This is not the same as “my friend takes 10 units, so I’ll take 10 units.”

Ten units of one concentration is not the same as 10 units of another concentration. A dose that is reasonable for one person may be too much for another. And with some peptides, the question is not only whether the dose “works.” It is also whether the dose is pushing the wrong pathway too hard.

Growth hormone-related peptides are a good example.

Peptides such as sermorelin, CJC-1295, ipamorelin, and other growth hormone secretagogues are used for body composition, recovery, sleep, and vitality. But these peptides affect growth hormone signaling, which is connected to IGF-1, blood sugar, fluid retention, tissue growth, and other metabolic pathways.

That does not mean they are “bad.” It just means they need to be used thoughtfully.

A medically supervised protocol may include lab review, health history, contraindication screening, dosing education, and follow-up monitoring. Without that, someone may be stimulating a pathway they do not fully understand.

Some Popular Peptides Have Limited Human Safety Data

Online peptide culture often talks about compounds like BPC-157, CJC-1295, AOD-9604, TB-500, or ipamorelin as if they are casual wellness supplements.

The regulatory picture is much more cautious.

The FDA has identified potential significant safety risks for several bulk drug substances that have been nominated for compounding. For example, the FDA has noted limited safety information and possible risks related to peptide impurities, aggregation, immunogenicity, and route of administration for certain peptides.

For BPC-157, the FDA has stated that it has no, or only limited, safety-related information for proposed routes of administration and lacks sufficient information to know whether it would cause harm when administered to humans.

For CJC-1295, the FDA has identified serious adverse events associated with use, including increased heart rate and systemic vasodilatory reaction, while also noting limited clinical data.

For AOD-9604, often marketed for fat loss, the FDA has noted limited safety information and possible serious adverse events, although causality is not clear.

In short, there’s no strong human safety data, appropriate dosing guidance, and medical monitoring. This isn’t to say peptides should be avoided, but they should only be used under the guidance and supervision of a health professional such as a functional medicine practitioner.

What Are The Serious Side Effects of Peptides?

Depending on the peptide, side effects may include:

  • Nausea, vomiting, diarrhea, constipation, or abdominal pain
  • Headache or dizziness
  • Injection-site reactions
  • Water retention or swelling
  • Changes in blood sugar
  • Increased hunger or appetite changes
  • Fatigue
  • Mood changes
  • Hormonal changes
  • Blood pressure or heart rate changes
  • Gallbladder or pancreas concerns with GLP-1 medications
  • Infection risk from poor injection technique or contaminated products

A 2024 study of GLP-1 receptor agonist cases reported to U.S. poison centers found that most cases had no or minimal effects, but a notable minority involved serious medical outcomes or healthcare facility admission. Reported cases increased during the study period, including an 80.9% increase from 2021 to 2022.

A later national poison-center analysis found that GLP-1 exposures rose sharply after semaglutide’s approval for weight loss and were accompanied by increased healthcare utilization.

That does not mean GLP-1 medications are unsafe when prescribed and monitored appropriately.

The “Peptide Stack” Problem

Another danger of using peptides without supervision is stacking.

In online wellness spaces, people may combine several peptides at once: one for weight loss, one for growth hormone, one for recovery, one for sleep, one for libido, and one for inflammation.

That can make it nearly impossible to know what is helping, what is causing side effects, or what is interacting with medications or underlying conditions.

A person may think, “I’ll just stop the one I started last.” But if several compounds are being used at once, the body’s response may not be that simple.

Medical supervision helps prevent this by asking basic but important questions:

  • What are we trying to accomplish?
  • Is this the right peptide for that goal?
  • Are there safer or better-studied options?
  • What labs should be checked first?
  • What medications or conditions change the risk?
  • What dose makes sense?
  • How long should this be used?
  • What side effects should prompt stopping?
  • What results are realistic?

That process may not sound as exciting as an online “biohacking protocol,” but it is much safer.

Who Should Be Especially Careful With Peptides?

Peptide therapy is not appropriate for everyone.

Medical supervision is especially important for people with:

  • A personal history of cancer or active cancer
  • Diabetes or blood sugar instability
  • Thyroid disease
  • Heart disease or uncontrolled blood pressure
  • Kidney or liver disease
  • Gallbladder disease or pancreatitis history
  • Autoimmune conditions
  • Pregnancy or breastfeeding
  • A history of severe allergic reactions
  • Multiple prescription medications
  • Hormone-sensitive conditions
  • Unexplained weight loss, fatigue, swelling, pain, or other symptoms that have not been evaluated

This is one reason a consultation with a health professional is critical. I don’t want to scare people away from every peptide; my goal is to make sure the protocol matches the person.

Online Advice Cannot Replace Lab Work

A major problem with DIY peptide use is that it often starts with symptoms, not data.

Someone feels tired, sore, inflamed, heavier than usual, or older than they want to feel. Then they see a peptide advertised as the solution.

But fatigue might be related to thyroid dysfunction, low iron, poor sleep, insulin resistance, menopause, stress, under-eating, overtraining, medication side effects, or an inflammatory condition. Weight gain may be related to blood sugar, cortisol, hormone changes, sleep, nutrition, alcohol, medications, or muscle loss. Slow recovery may be related to protein intake, training load, nutrient deficiencies, or inadequate sleep.

Without evaluation, peptides can become a very expensive guessing game.
