A few days before that time of the month, I joke with my husband, Preston, that maybe he should grab a hotel room. Just for a night or two. Nothing personal. It’s just… safer for everyone. Now, I personally don’t think I morph into some hormonal monster—no Dr. Jekyll to Mrs. Hyde situation here. I’m steady Eddy all month long… right, babe? Haha.
But for many women, PMS isn’t just a little crankiness and a craving for chocolate—it’s next-level. We’re talking rage texting, crying over a gum commercial, and fantasizing about punching your coworker in the throat.
When it’s that intense, we’re no longer talking about regular PMS. We’re talking about Premenstrual Dysphoric Disorder, or PMDD. And today, we’re going there. I’ll walk you through what PMDD is, why some women are more prone to it, what conventional medicine says, and how functional medicine can help you kick it to the curb.
What Is Premenstrual Dysphoric Disorder?
PMDD is basically PMS’s evil twin. It’s a severe, sometimes debilitating form of premenstrual syndrome that affects about 3–8% of women of reproductive age. While PMS might make you irritable or bloated, PMDD can completely hijack your mood, motivation, and mental clarity.
The term “PMDD” was officially recognized in 2013 when it was added to the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), but let’s be honest—women have been living it for way longer than that. Only recently has it started trending, especially with more awareness about hormone-driven mood disorders on platforms like TikTok and Instagram.
Symptoms show up during the luteal phase of the menstrual cycle (about a week or two before your period) and often disappear once your period starts. But in the meantime? You might feel anxious, depressed, angry, foggy, and just plain not like yourself.
Why Do Some Women Have PMDD?
Like most things in functional medicine, it’s never just one thing. Here are a few root causes:
- Hormonal imbalances – especially low progesterone or poor progesterone-to-estrogen ratio.
- Poor estrogen detox – if your liver isn’t clearing excess estrogen well, it can cause mood and bloating chaos.
- Inflammation – systemic inflammation makes everything worse, including hormone sensitivity.
- Nutrient deficiencies – magnesium, vitamin B6, zinc, and omega-3s all support hormonal and neurotransmitter balance.
- Gut issues – yep, the gut-hormone connection is real. Poor gut health = poor estrogen metabolism = mood mess.
- Genetics and sensitivity to hormone fluctuations – some women are just more sensitive to hormonal changes than others.
Are There Medicines for PMDD?
Yup. Conventional treatments include SSRIs (antidepressants), hormonal birth control, and in severe cases, even GnRH agonists that essentially put you into temporary menopause. These can help some women, but let’s be real—they don’t address the root cause and may come with their own side effects.
And if you’re like many of the women I see, you’re tired of just slapping a band-aid on it. You want to feel normal again—without numbing your brain or tanking your libido in the process.
Overcoming PMDD: A Functional Medicine Perspective
Let’s talk natural solutions. One of my favorite tools? Bioidentical progesterone cream. For many women, PMDD is rooted in low or unstable progesterone levels during the luteal phase. Progesterone is your calming hormone—think natural chill pill.
Here’s how we use it:
- When to take it: During your luteal phase (roughly days 14–28 of your cycle, depending on when you ovulate).
- How much: Most women start with 20–30 mg of bioidentical progesterone cream applied to the inner arms or thighs.
- When to stop: You stop on day 1 of your period, then repeat the next cycle.
Always work with a provider to customize your dose based on your labs and symptoms. This isn’t one-size-fits-all—but when it’s right? Game changer.
I may also recommend targeted supplements (like magnesium glycinate or B6), liver support for estrogen detox, and gut healing protocols. Your PMDD isn’t just a hormone issue—it’s a whole-body signal something’s out of alignment.
The First Step to Conquering PMDD: Lab Tests
Want to stop guessing and start fixing this? It starts with lab testing. I run a comprehensive hormone panel so we can see where your progesterone, estrogen, testosterone, cortisol, and other key markers are at. Once we have your baseline, we create a plan based on your body, not a generic prescription pad.

And if you’re ready to stop dreading half your month, book an appointment with me and we’ll meet at the Diabetes & Wellness Clinic in Norfolk. I see women from all over northeast Nebraska, and yes—we take insurance. Because you deserve to feel calm, steady, and like yourself all month long. Let’s kick PMDD to the curb—with science, strategy, and just the right amount of sass.
