One of the more annoying things about midlife is that your body suddenly starts changing the rules, even if you’re eating the same, doing the same exercise routine, and eating relatively healthy, not living on gas station pastries and margaritas.
But then you go for your annual physical and blood work and your lipid panel comes back totally out of whack.
Another reason to hate your hormones.
You see, for many women, perimenopause and menopause bring changes in cholesterol, triglycerides, blood sugar, body composition, sleep, stress tolerance, and cardiovascular risk. That does not mean every woman suddenly needs to panic over their labs.
However, it also does not mean we shrug and say, “Well, hormones are weird,” and move on.
This is where I straddle the line a bit.
As a Nurse Practitioner, I am trained to recognize that a total cholesterol score over 200 mg/dL is considered high. That is the conventional medical cutoff.
But when I put on my functional medicine practitioner hat, I am usually not overly impressed by total cholesterol by itself.
Why?
Because total cholesterol is a total number. It is a little like body weight.
If someone tells me they weigh 170 pounds, I still do not know much. Is that person muscular? Inflamed? Sedentary? Strong? Metabolically healthy? Losing muscle? Gaining fat? Holding water? Training like a beast?
Same with total cholesterol.
A total cholesterol number gives us a broad snapshot, but it does not tell us what is really happening under the hood. And at midlife, under the hood is exactly where we need to look.
Cholesterol Guidelines Keep Changing
Part of the confusion is that the definition of “normal” has changed over time.
Years ago, cholesterol numbers that might have been considered less concerning may now trigger a longer conversation about cardiovascular risk. This does not mean your body changed overnight. It means medical guidelines changed as research evolved.
That can be frustrating, but it is not automatically a bad thing. Guidelines change because researchers continue learning more about heart disease risk, population patterns, inflammation, plaque development, insulin resistance, and long-term outcomes. (I know, I know, you’re probably thinking that BIG PHARMA is greedy and that’s why they keep lowering the ranges; that may be a part of it, too.)
The problem is when patients are handed a number without context. The data on a lipid panel (triglycerides, LDL, HDL, total cholesterol, etc.) needs to be interpreted with hormones, metabolism, family history, lifestyle, inflammation, and menopause status in mind.
Why Menopause Can Shift Your Lipid Panel
Estrogen does more than regulate your period. It also plays a role in vascular health, insulin sensitivity, fat distribution, inflammation, and lipid metabolism.
As estrogen declines during the menopausal transition, many women see changes in their lipid profile. Research from the Study of Women’s Health Across the Nation (SWAN) found that lipid changes tend to occur most noticeably during the later phases of the menopause transition, with increases in LDL cholesterol and other lipid markers.
Other research has found that postmenopausal women may have higher total cholesterol, LDL cholesterol, and triglycerides, along with lower HDL cholesterol, compared with perimenopausal women.
Your cholesterol may change even if your habits have not changed dramatically. That does not mean hormones are the only reason. Age, sleep, muscle mass, stress, thyroid function, alcohol, insulin resistance, and diet play a role. But menopause can absolutely shift the goalposts.
The Lipid Numbers I Care About More Than Total Cholesterol
Total cholesterol is not useless. It is just incomplete.
When I review a lipid panel, I want to see the bigger picture, including:
- Triglycerides: These are fats in the blood that often rise with excess sugar, refined carbohydrates, alcohol, insulin resistance, and metabolic dysfunction. High triglycerides can be a clue that your body is not processing fuel well.
- LDL cholesterol: Often called “bad cholesterol,” LDL carries cholesterol into circulation. LDL is not evil. Your body needs cholesterol. But when LDL is too high, especially in the presence of inflammation, insulin resistance, or high blood pressure, it can contribute to plaque buildup.
- HDL cholesterol: Often called “good cholesterol,” HDL helps transport cholesterol away from tissues and back to the liver. But HDL is not a magic shield. A high HDL number does not automatically erase other risks.
- Non-HDL cholesterol: This is total cholesterol minus HDL. It gives us a broader look at the cholesterol particles more likely to contribute to plaque.
- ApoB: This measures the number of atherogenic particles, meaning particles that can contribute to plaque formation. Some people can have “normal” LDL cholesterol but still have a high number of these particles.
- Lp(a), or Lipoprotein(a): This is a genetically influenced cholesterol particle that is not included in a standard lipid panel. More on this in a minute….
This is why I do not love the “your cholesterol is high” conversation when it stops there.
Which part is high? How high? Compared with what? What are your triglycerides? What is your HDL, non-HDL, and your family history? Also, what do your A1C blood sugar levels look like? How about your blood pressure and overall inflammatory picture? Do you have insulin resistance?
Do you see how holistic cholesterol and lipids really are? I wouldn’t tell you, “Hey, your total cholesterol is 220. Here’s an Rx for a statin. Good luck!”
Lifestyle Choices That Can Help Improve Lipid Markers
The good news is that many lipid markers respond well to lifestyle changes. Not always dramatically, but often enough that it is worth taking seriously.
Here are the big ones….
Reduce Simple Carbs and Sugar
Triglycerides often rise when the body is getting more sugar and refined carbohydrates than it can efficiently handle.
This does not mean you can never eat bread again.
But if triglycerides are high, one of the first places to look is added sugar, sweet drinks, desserts, refined grains, and frequent snacking. Improving blood sugar regulation can often improve triglycerides and support healthier cholesterol patterns.
Reduce Alcohol
Alcohol can raise triglycerides, interfere with sleep, stress the liver, worsen hot flashes in some women, and make midlife weight management more annoying than it already is.
Again, this does not mean everyone needs to live like a monk. But if your triglycerides are elevated, your sleep is trash, your hot flashes are flaring, and your lipid panel is moving in the wrong direction, reconsider having half a bottle of wine every night.
Exercise Consistently
Exercise supports insulin sensitivity, blood pressure, HDL cholesterol, triglycerides, body composition, mitochondrial function, and mood.
For women in perimenopause and menopause, strength training is especially important (even more so if you’re taking a GLP-1 medicine). Muscle is metabolically active tissue. The more muscle you maintain, the better your body tends to handle glucose, lipids and aging in general.
Optimize Sleep
Poor sleep can worsen insulin resistance, cravings, appetite regulation, blood pressure, inflammation, and stress hormones. All of that can indirectly affect your lipid profile.
And yes, menopause can wreck sleep patterns. Night sweats, 3 a.m. wakeups, anxiety spikes, and temperature changes are real concerns.
Manage Stress
Chronic stress can affect cortisol, blood sugar, blood pressure, sleep, inflammation, food choices, and recovery.
You do not need to meditate like a Buddhist monk to calm your body. But your nervous system does need regular signals that you are not being chased by a bear, a deadline, your inbox, and your family calendar at the same time.
Stress management can include walking, strength training, breathwork, therapy, prayer, time outside, better boundaries, or simply not saying yes to every soul-sucking obligation that crosses your path.
The Lp(a) Test: The Lipid Marker Many People Have Never Checked
Lipoprotein(a), or Lp(a), is one of the most important lipid markers that is often missing from routine testing.
Lp(a) is mostly genetic. You do not develop high Lp(a) because you had too much pizza over the last few months. You usually inherit it.
The American Heart Association describes Lp(a) as a cholesterol-carrying lipoprotein that can increase the risk of heart disease and stroke when elevated. About 1 in 5 people have high Lp(a), and many have no symptoms.
This is why I like testing it at least once.
If your Lp(a) is high, it does NOT guarantee you will have a heart attack or stroke. But it does mean your baseline risk may be higher, especially if other risk factors are also present. Think smoking, high blood pressure, insulin resistance, inflammatory conditions, family history, elevated LDL, poor sleep, or being overweight.
In practical terms, a high Lp(a) can function like carrying an extra cardiovascular risk burden. You may be doing a lot right and still have lipid numbers that do not look ideal. Again, this has nothing to do necessarily with lifestyle factors. There are some people who are doing everything right but just have a genetic inclination to have high Lp(A).
And it’s genetics where functional medicine should not get fluffy (no pun intended with lipoprotein particle size). What I mean by that is functional medicine does not guarantee that herbs, supplements, and “clean eating” fix everything. Instead, functional medicine uses the best of Western diagnostics, deeper testing, lifestyle medicine, and individualized care.
So yes, we can run advanced labs and look at genetics, and, as a certified health coach, I can help you optimize your nutrition, exercise, sleep, stress, hormones, and inflammation.
But if your genetic risk is high enough, we may also need to talk about medication, which includes the “S” word: statins.
When Are Statins Necessary?
Statins are not automatically necessary because your total cholesterol is over 200.
But they may be appropriate when your overall cardiovascular risk is high enough.
Conventional guidelines generally consider statins more strongly when someone has very high LDL cholesterol, diabetes in certain age groups, existing cardiovascular disease, or an elevated 10-year risk of cardiovascular disease. Current prevention guidelines also consider “risk-enhancing factors,” which can include family history, metabolic syndrome, premature menopause, inflammatory conditions, and elevated Lp(a).
A woman with mildly elevated total cholesterol, great triglycerides, strong HDL, low ApoB, normal blood pressure, excellent blood sugar, low inflammation, no family history, and a healthy lifestyle is not the same as a woman with high LDL, high ApoB, high Lp(a), insulin resistance, hypertension, and a father who had a heart attack at 52.
Are Statins Risky During or After Menopause?
For many people, statins are well tolerated, but they can cause side effects in some people, including muscle aches, digestive issues, fatigue, brain fog, or changes in liver enzymes. Serious side effects are uncommon, but they do happen. Mayo Clinic notes that if side effects occur, changing the dose or switching to a different statin may help.
So no, I do not think every perimenopausal or postmenopausal woman with a borderline lipid panel should automatically be put on a statin.
But I also do not think statins should be treated like some sort of pharmaceutical villain hiding in the bushes.
One important note: statins do not lower Lp(a). The American Heart Association’s Lp(a) toolkit notes that statins are ineffective at reducing Lp(a) and may modestly increase it in some cases. That sounds alarming, but statins can still lower LDL cholesterol and reduce overall cardiovascular risk. They may also help stabilize plaque, which is one reason they are commonly used in people at higher cardiovascular risk.
So if Lp(a) is high, the goal is not lowering Lp(a) with a statin. Instead, the goal is reducing the rest of the risk burden around it. In other words, if your genetic risk is elevated, we want fewer other problems piling onto the same fire.
High Cholesterol & Menopause: The Bottom Line
Menopause can change your lipid panel. But that does not mean you are cardiovascularly doomed or that you did something wrong. It also does not mean that one total cholesterol number tells the whole story.
But if you’ve noticed a spike in your blood panel lipid scores, we need better context.
A standard lipid panel is a starting point. A deeper cardiovascular risk picture may include triglycerides, LDL, HDL, non-HDL cholesterol, ApoB, Lp(a), blood sugar markers, blood pressure, inflammation, thyroid health, menopause status, family history, and lifestyle patterns.
If your cholesterol changed during perimenopause or menopause, do not panic. But do not ignore it either.
Get the right data. Let me help you ‘look under the hood; and we’ll make a plan that fits your actual risk.
That may include nutrition, strength training, sleep support, stress reduction, hormone conversations, advanced testing, and yes, when appropriate, medication (sometimes doing 100% natural doesn’t work!).
This is the essence of functional medicine, which is good medicine.
